HMIS Privacy Policy
To best assist you, we use the Lane County HMIS database to manage our intake assessments. Your information is visible to a limited number of Lane County social service providers and protected using the highest security standards. Law enforcement and DHS will not have access to this system. This information allows us to understand community needs and advocate for further services. If you have any questions or would like a copy of this form, please let me know. HMIS permits photo IDs, we request participants allow their photo to be taken to create an ID badge.
The ESS Navigation Center program in a Lane County project. We may share program and client information with Lane County staff, funders, and any OHCS members. Contractual obligations signify Lane County staff, funders, and OHCS members have the right to retrieve client data/files at any time for review.
Personally identifiable information is protected by federal laws (Privacy Act of 1974, as amended and will be collected for the purpose of determining program eligibility, providing assistance/service, data collection, reporting and monitoring. Personally identifiable information will be shared with Oregon Housing and Community Services and other state agencies with an information sharing agreement with OHCS and are administering programs that serve the same or similar clients or populations, as is necessary to carry out the intent of an assistance or service program for the benefit of the person applying for such assistance or service and will be disclosed to Oregon Housing and Community Services without written authorization.
